Our Transcriptionists, Editors, and Management Staff are bound by a Confidential Non-Disclosure Agreement.

QUALITY:

Prolonged and consistent investment in training and IT systems has led the way to meeting our high standards.

All our medical transcriptionists receive a minimum of 3 months in-house training before transcribing for our clients without the direct supervision of a Senior Transcriptionist. All transcriptions are checked word for word by a Senior Transcriptionist before being delivered. We use lexicons, dictionaries, directories and in-house lists incorporated into our system by our skilled IT programming team. Our own in-house control and reporting system, monitors files and quality on a daily basis, highlighting at an early point any issue or queries for resolution. Most importantly, we have a dedicated Quality Control Department that oversees all aspects of our medical transcription and has access to every process at all levels. Our QC Department is tasked with signing off every transcript before delivery; no file is delivered to our clients without review by our QC Department.

The Quality Control Department's sole task is to validate and maintain the high level of quality that Horizzon has set as a standard for ALL transcripts.

Horizzon follows standards established by the AAMT to ensure a high accuracy rate for all transcribed reports submitted to your health care organization. Horizzon's Quality Control Program is based on standards derived from the following reference materials:

  • Stedman's Medical dictionary.
  • Stedman's Medical Word Books.
  • The Journal of the American Association for Medical Transcription.
  • Formats and Style information provided by Clients.
  • Quality Assessment Procedure

Horizzon's Quality Control is as below:

1. Random Quality Review:
Every employee has an ongoing random quality review. The Medical Language Specialists (MLS) monthly production is reviewed and checked against the soft copy of the reports with the voice recording. Errors are then marked on the soft copy according to Horizzon's Quality Control Criteria. Quality reports are submitted to the QA Manager each month for evaluation. The Monthly Quality Review Form is used to document the errors and calculate the percentage of accuracy. Horizzon's quality standard is 99% accuracy. Each transcriptionist is expected to reach a Minimum average accuracy of 94%. Transcriptionists who achieve an average quality rating of 95% or above for three consecutive months are eligible to be a Proofer. Proofers who achieve an average quality rating of 98% or above for three consecutive months are eligible to be an Editor.
